I've been using SurveyMonkey for quite a while now and have no problems with it. Nice reporting features and the ability to let others view your survey results or reports through a special link you create is nice as well.

My choice is Zoomerang. I started with their free account and now have a pro account. They have all I need and I have never had any problems, unlike my free account to try out SurveyGizmo.

The SurveyGizmo site needs a re-working of the cookie delivery (no need for the separate cookies PER PAGE, just one cookie would work)
