This'll probably sound snakey, but… I inadvertently stumbled upon a fine solution for Outlook users yesterday, as I was snooping around my own site and spied a button sponsor. I use POPFile. I have 3 email accounts and 7 different mail folders ranging from spam to individual email account folders.
93% accuracy rate with all of this on my computer. ;-)

Spambayes Outlook Plugin, a nice free open-source plugin, works great for me, and kills 99% of my mess.

I use this service called knowspam (knowspam.net) – its a proxy server that filters out spam. The great part is that by default, all mail is blocked unless on an allowed list. Users can send their mail through by self-validating themselves as “human”. Its blocked something like 14,000+ spam mails in the 2 months or so I've been using it.

I switched to Opera and M2 after an XP update made OutlookExpress crawl like molassis in winter. I've been pleased with nearly everything about the browser and email client. It's spam filter is about 95-99% for me. IMHO Opera is #1.

I've been using IHateSpam for Outlook (also available for Outlook Express) for about a year now. I like it a lot 'cause it uses many methods for blocking including community reporting and black and white lists, etc. Check it out – http://www.sunbelt-software.com/product.cfm?id=930 and no I don't work there or anything. I just like it.
