Yep unfortunately you are finding the same thing that every other "interactive semi open conference call / podcaster - managed but not entirely anarchy" talk show is finding that some of the tools are out there but it's true 'art' to manage these tools.

Some people like Jerry Michalski for the weekly Yi-Tan call are masters at running a single person host/producer whilst most other mere mortals require two people to make this work properly.

There are a number of Asterisk based providers out there - if you want to build your own truly custom solution and want a mix of landlines and voip can I suggest that you check out www.Mexuar.com as your voip interface.

Basically Mexuar Corraleta is a Java Applet that allows any web page to become a softphone.

This means that people are able to browse to your site to join the live conference call -   click the java applet and using their headset/mic and browser place a free call to your asterisk server wherever in the world they may be listening from.

Using Asterisk of course it's easy to build a dtmf control - ask questions (which can be diverted to the producer etc) or voting applications thumbs up thumbs down etc.
